This equipment is the property of Florida International Nicole Wertheim College of Nursing and Health Sciences (FIU CNHS) and is loaned to me for my use as long as I remain a faculty member at FIU CNHS. Unless indicated as DISBURSED EQUIPMENT, I agree to the following terms upon receipt of the Equipment: The equipment has been issued to me in working condition. The equipment shall be returned in the same condition as was issued to me. It will be delivered to the Department of Information Technology (AHC3-206) within 24 hours, or the set date as agreed upon. If the equipment is lost, stolen or damaged beyond normal wear and tear, I agree to reimburse the College of Nursing and Health Sciences the original or depreciated cost of the Equipment.
