Once you complete the form, it will go to our system and you will receive an email within 24-48 hours with your appointment time, date and instructions to connect via ZOOM conferencing with your advisor and other details that you may need.

Please do not submit duplicate appointment requests because it will delay scheduling your appointment.

For CPT, OPT, OPT STEM extension, Economic Hardship applications: please upload your application documents on your FIU ISSS Portal (under F-1 Practical Training Applications). For Medical Reduced Course Load approvals (under F-1 Reduced Course Load Request). After you upload your documents, you will be able to request an appointment in the ISSS Portal.

PLEASE NOTE: If you are out-of-status and will TRAVEL to regain your F-1 student status, then please submit a RESTART request in the ISSS PORTAL > Document Request > Restart to F-1 Status > Complete STEP 1 and STEP 2. [No appointment is required] *
